Appointments
If you would like to make an appointment, please contact our office by calling (863) 802-8855. If you would like to request an appointment online, please select the "Appointment Request" option from the menu above. A member of our staff will follow up with you directly to confirm scheduling.
Financial
For your convenience we accept Visa, Mastercard, American Express, personal checks and cash. We are “In-Network” with some insurance plans including: Blue Cross/Blue Shield, Florida Blue, United Healthcare, Humana, Medicare and Medicare Advantage Plans such as Medicare Complete and Devoted. Please call with your insurance card number and we will be happy to verify your specific coverage. For more information regarding payment options at our practice, please click here.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
